Financial
Policies
For
our patients we have adopted the following financial policies. If you
have any questions, regarding these policies, please contact our Patient
Account Representative.
We are dedicated to providing the best possible care and service to
you and regard your complete understanding of your financial responsibilities
as an essential element of your care and treatment.
Unless other arrangements have been made in advance by either you or
your health insurance carrier, full payment is due a the time of service.
For your convenience we accept VISA, MasterCard, Discover, and American
Express.
Your Insurance
We
accept an assignment of benefits with most insurance plans (HMO, PPO,
POS, etc.) and with Medicare. This means that we bill
those plans for which we have a contract with and will only require
you to pay the authorized co-payment at the time of service. Please
contact us to verify if we accept your particular insurance plan.
If
you have insurance coverage with an insurance plan for which we do not
have a contractual agreement, we will provide you with an itemized statement
of all charges for you to file with your plan. Your insurer will send
payment directly to you; therefore, the charges for your
care and treatment are due at the time of service.
